If you put "SAP creating master records for material determination" into your favorite internet search engine, you'll get the hep you need to set up an automatic process to get the system to offer an alternative material to the one entered in the sales order entry process. Transaction code is VB11.

If you just want to 'reference' the new material to the replaced one (you said 'Is there any way we can look up a Replacement Item # for Discontinued Item # is SAP?') then one way* to do this is to just put the new material's part no into the description of the old part no. (keeping the old description somewhere else in the old material master, such as basic data text, for reference). *How to do this depends on how you control the (non-)availability of the superseded material in your system (eg material statuses, etc).
